The steps I'm using:
1.Create overworld/outline using level gen.
2. Create storyline with a detailed plot and characters (I'm on this part for a playerworld I started that I work on in my spare time)
3.Write out some main towns and what they will do in the storyline and draw a rough sketch of what they'll look like and start work on storyline quests.
4.Build the main towns and add NPC's that move about the town doing things randomly that gives it a touch of realism.
5.Add some small towns that aren't necessary for the growth of the story but are fun or can be used to collect an item to start a quest or such.
6.Start major work on quests and make sure they fit in with the plot/storyline.
7.Add house insides that fit with the outside of the house,like don't make a giant steel hosue and then have the insides look like it is made of plants.
8.Add in some small things to do so the players don't gt bored after they finish the main stuffsuch as fishing,etc.
9.Let some friends test it and tell you what needs to be fixed and what doesn't fit/make sense.Fix those and then on to step 10!
10.Submit it to Genmo or TrueHeaT and if it makes it cry about how long it's taking to go on the server list.
