Thats actually a good idea, why dont we have it? A world without rules, npc's or whatever. Just your sword, the pause button, and a large gmap full of PKing goodness

A few servers that were pretty much like this have been tried (i.e. Graal 3000) but they didn't really go anywhere.

I've said it once and I'll say it a hundred times.... no playerworld can succeed without Project Management. Graal itself doesn't have project management, yet somehow manages to cling to existence. It's pretty stunning.
