Obviously they should have a chance to say their side of the story so you can make a fair judgment but if you have seen them walking over walls or something similar just ban them.
However when you do ban players you should put clearly in ban comments why they are banned, the date and your name.
Always the ban reason goes in ba comments, other stuff can go into their normal commeents I just find it more efficent to put it into ban comments.
